Client Agreement

  • Treatment

    I understand that Active Release Techniques (ART) is provided for relaxation, relief from muscular tension, improvement of circulation, and increase in range of motion. I understand that my provider is not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat physical or mental illness. I understand that massage is entirely therapeutic and non-sexual in nature. If I experience pain or discomfort during the session, I will immediately inform my ART Provider so that pressure/strokes can be adjusted to my level of comfort. I will not hold my provider responsible for any pain or discomfort I experience during or after the session.

  • Voluntary Consent

    I give my consent to receive ART and/or massage services and/or other bodywork or treatment and/or stretches and/or exercises (the “services”) from Asher Sports Therapy, and I acknowledge and agree that I am doing so at my own risk. My health and safety with respect to such services are my sole responsibility. I acknowledge that my receipt of the services from Asher Sports Therapy may result in bodily injury to me or my death. My decision to receive services from Asher Sports Therapy is voluntary, and I know of, understand and assume any and all the risks associated therewith. I waive and release my therapist, Asher Sports Therapy, and all staff, affiliates, contractors from any and all liability, past, present, and future relating to massage therapy and bodywork and sports therapy and exercises.

  • Medical Care

    I understand that the services offered today are not a substitute for medical care. My physician approves of the therapy and has taken X-rays to ensure that there are no existing injuries and that the treatment will not cause further damage.

    I understand that, if I have not gotten approval from my physician, that I acknowledge that the therapy has the potential to aggravate any existing problems or injuries.

  • Full Disclosure

    I have notified my therapist of all known medical conditions and injuries. I agree to inform the therapist of any changes in my health and medical condition. I understand that there shall be no liability on the therapist’s part should I forget to do so.

    I understand that any information provided is for safety purposes and will be kept strictly confidential, except that such information may be used for statistical analysis or scientific purposes.

  • Waiver of Liability

    In exchange for receiving services from Asher Sports Therapy, I, for myself and on behalf of my heirs, executors, administrators and personal representatives, hereby waive, release, discharge and hold harmless Asher Sports Therapy, its members, officers, employees and agents from any and all liability for any and all injuries, including death, damages or claims relating to or resulting from my receipt of the services, now or in the future, foreseen or unforeseen. Further, I will indemnify and hold Asher Sports Therapy, its members, officers, agents and employees, harmless from and against any and all claims, rights, damages, liabilities, losses, costs and expenses (including reasonable attorneys’ fees) arising from or in connection with any injuries to other persons or damage to property caused by or attributed to me.

  • Permission to Use Content

    I grant to Asher Sports Therapy and its representatives the right to take photographs, videos, and audio recordings of me and my property in connection with my athletic activities and therapy. I authorize Asher Sports Therapy and its assigns and transferees to copyright, use, and publish the same in print and/or electronically. I agree that Asher Sports Therapy may use such content with or without my name and comments and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.

  • Cancellations and Missed Appointments

    I understand that, in order to provide sufficient time for all of their clients, a 24 hour cancellation notice is required for all scheduled appointments. If I fail to give 24 hours notice and miss or cancel an appointment, the full fee for the scheduled service will automatically be charged to my account and credit card on file.

    I understand that if I arrive late, my session will end at the original scheduled time so the client following me is not penalized.

Privacy Policy

  • How We Use Your Info

    As a client, you authorize us to reasonably use photographs, videos, and audio recordings of you for promotional and educational purposes, in print, on our Website, and on social media. At any time, you may request that we not use this information. However, we do not share or sell your personal information with any outside parties without your permission. This includes your address, phone number, email address, personal health information, and credit card information.

  • Data Security

    Our leased servers are located in a state-of-the-art datacenter that is equipped with multiple layers of network security. They have hardware DDoS protection (Distributed Denial of Service attack) and are monitored 24/7/365 by system administrators to ensure reliable uptime and data security. All sensitive data is encrypted during transmission, including client-Website and employee-Website connections.

  • Credit Card Information

    We never see your credit card information or store it in our system. When you enter your credit card information in a form on our Website, the data is sent in an encrypted connection directly from your browser to our payment processor, where it is stored on their secure PCI-compliant servers. They provide us with the last 4 digits of the card number a "token" (made up of random characters) that identifies your account. We store the token in our system and use it when we need to have our payment processor charge your credit card.

  • Health Information

    We respect your private health information and follow the Health Insurance Portability and Accountability Act of 1996 (HIPPA) . Access to your treatment notes and other health information is restricted to our providers and system administrators. We will ask you to complete written authorization before we use or release your health information if our use or disclosure is not for the purposes of treatment, obtaining payment, or maintaining our systems. You may withdraw your authorization from us at any time in writing to ensure that we will not share your health information in the future.
